The patient’s doctor and the hospice medical team will work together with the patient and family to set up a plan of care that meets patient’s needs. The plan of care to provide Hospice Care in Central Ave Riverside California includes hospice services that are covered by Medicare/Medicaid or private insurance, and a specially trained medical team and support staff will assist in pain management, emotional, social, and spiritual support.
The patient and family members are the most important part of the hospice team and are included in the decision making process. The team will also include some or all of the following people:
- Doctor
- Skilled Nurse
- Social Worker
- Chaplain
- Hospice Aide
- Trained Volunteer
- Bereavement Support
In addition, we provide on-call 24-hours a day, seven days a week to give our patient and their family support and care when they need it.
Although a hospice doctor is part of the medical team, the patient’s regular doctor can also be part of this team. A nurse practitioner may serve as the attending care professional when requested by the patient. However, only the patient’s doctor and the hospice physician can certify that the patient is in terminally ill condition. The hospice benefit allows the patient and their family to stay together in the comfort of their homes. If the hospice team determines that an inpatient care is needed, the hospice medical team will make the arrangement for the patient’s stay.
