- Physician plays a key role by diagnosing, evaluating, and working with the team to develop a care plan for each patient to address pain symptom and management.
- Skilled Nurse is experienced in assessing and monitoring patient’s medical care and medication that are necessary to ease their pain and keep them comfortable. They provide hands-on care and are the link between the patient, their family, and the other hospice team members.
- Social Worker is a trained professional who counsels and advises the patient and family members with the non-medical needs that arise during the course of care. He/she can assist with final arrangements and act as community advocate, making sure the patient has access to the needed resources.
- Chaplain provides emotional and spiritual counseling to the patient and family members as they cope with end-of-life stage. Spiritual care is a personal process that can give a peace of mind and emotional strength to both the patient and family.
- Hospice Aide is specialized and trained to assist with personal care such as bathing, dressing, meal preparation, and light housekeeping.
- Trained Volunteer is caring individual who offers the patient and family members compassionate support through friendship and listening ears. They can assist with everyday tasks such as reading, shopping, and carpooling.
- Bereavement Support enters the care plan at the time of mourning and helps the family through the grieving process. Support may include loss counseling as well as memorial service.
Arrangement will be made for hospice approved medications, medical supplies, medical equipment, as appropriate.
